Does this include VAT? And what rate is VAT in Italy?

Wich means that is going to be the price in Italy. Eventually lower but just few euros.
Normally retailers as FCF and DPF in Milano sell at little lower prices than Canon suggested price. And if you are a pro and know those retailers well you can always get a small discount.
From DPF, I was able of buying a a Mamiya7 when it was at its highest demand, for almost 10% less than advertised than Mamiya. This wont be the case of the Mark two, but if Canon Italia says 4,391, it will not be more than that.

Andrea, I got my old 1D at quite less than the list price (Canon Italia warranty), ditto if I had to buy now the 1Ds, which I can get at 15% less than suggested Canon Italia price from two different resellers.
This doesn't mean that the beast is cheap... on the contrary, 8400 € for the 1Ds is an awful lot of money, actually twice the price of the 1D mkII !
Let's hope they will lower its price within a month or two...
