I never found any use for a monopod with a ball-head. If you switch to a tilt-head, you aren't gaining anything. I'd consider that first.

I think those are a good, heavy duty monopod, but I know I'd never use the ball-head.

I have never used a monopod. But let's say I am up on the stands shooting , how can I make sure the camera is facing down without straining shoulders ? Ball heads I assume allow you to lock the camera on any angle

I'm not sure what you mean. A tilt-head on a monopod is a lot like a gimbal on a tripod. Once the monopod is in position, you just pan and tilt to target the subject. You don't need to lock it in use. It's just a steady rest and pivot point. That's why I don't like the ball-head.

When I've used a ball-head on a monopod, it was always to get in some odd, awkward position, but it's not much different than hand-held.

To support the weight and maintain level, the monopod needs to be directly under the gear. The tilt-head naturally does that.
