Weird noise after reduction?

So I'm still new to playing with my 60D, and Aperture, LR, and CS5 (still deciding which I like best). Anyways I was playing with the Nik Software Dfine noise reduction plug in, and after running it these white pixels started to pop up (look in hair, around zipper, black background, etc). They never showed up in the preview, but after I clicked save, and it did it's thing, these came around. Any ideas on what I did wrong? I'm sure it's 99% user error, but after playing for a while I'm stumped and thought I'd ask the pro's.



 
This is a B&W conversion , presumably the white pixels were originally coloured ? They could be hot pixels which can occur if several pictures have been shot or framed in low light with relatively long exposures , this is caused by the sensor over-heating.

Would be of interest to see the complete workflow, since Dfine in general should be applied first, before working on the image and especially before converting to bw.

But Keiths explanation might be the one. A detailed look at the original file should clarify this issue hot pixel / dead pixel.

Thanks guys. Yes I did Dfine first in my work flow, and then converted to B&W and cropped to taste, though I did bring the exposure up one stop after importing. Then I cloned the edge of my cases out as my last step. This shot was a very very dark shot of my wife standing in my world on my last tour, so the lighting is definitely constant (music industry).
 
One proposal to check where the white pixels come from: if you copy the bw version as a second layer on top of the original and reduce the opacity of the top layeryou may easily find out the original pixel to see if they where there already but in a different colour.
