I have used mainly PSP 7 exclusively for most of the past 18 months. I love its red-eye removal tool, and find its interface much easier to work with.
I only upgraded to Photoshop 7 recently after having purchased Peter iNova's ebook on the F707. Since purchasing that book, his PS tutorials are the first I've run into that make life easier for someone like me (who has been totally intimidated by Photoshop). I don't like PS Elements for the same reason.
Anyway, I agree with you about Neat Image, but I feel it's performing its best when you have a way too noisy image (higher ISOs). I tried it on this image just to see what it would do with it. Even the publisher of Neat Image says that the software is better with severe problems.
I, too, like PSP's edge preserving smoothe tool. I also like using the median filter for noise removal. No matter which way you go, noise removal is inevitably going to give things a waxy look, it just comes down to how much of a trade-off you want.
Peter iNova's got some PS actions built into his ebook, and there's one dealing with noise. I can't wait to try one of them out.

I made some changes to your image. Let me know what you think.
I did the following:
1) Ran the image through Neat Image to remove the noise;
2) Ran the unsharp mask in PS7 with the following settings:
Amount: 300%, Radius: 0.5 Pixels; Threshold: 1 Level; and,
3) Increased saturation by +25.
