Indoor Shooting help

Hello I need some help on how to correct the circled issue blotch and how to correct how grainy my photos come out.  I am using natural lighting combined with studio lights. I am using a Canon T5I with a Sigma 17-50mm lens. I already have the sharpness set near the max but when shootinf I always experience these grainy spots. Thanks for any help.

I'd get the ISO to under 1000.
You don't need f16, maybe a lower f number -- 5.6 or 8.0 ?
Use more light if needed.
