It's ignorance, weakness, your own deliberate fault. But it's not an accident.

Changes a lens. Your grip slips on the camera. Try to catch it before it falls. Fingers go in the lens mount and hit the shutter.

I could see a path to an accident easily enough that doesn’t require disparaging the unfortunate person who had it happen.

So can I, but it would be very charitable to call it an accident waiting to happen. Changing lenses on a table, well away from the edge or with the camera on a strap around the neck or wrist would avoid that. The consequences for a similar disaster with a DSLR would be even worse. Though that risk would be minimised if the mirror locked in the up position the instant the lens release was pressed and not come down until the DSLR detected that a lens had been fitted successfully. I can't remember that feature ever being suggested though.
