I shoot for several organisations and on one occasion I was taking a lot of shots of static items using a tripod. I found quite a lot of OoF shots, which was unusual, but I couldn't work it out at first & then the penny dropped! I'd forgotten to switch off the IBIS!

When I returned with the IBIS off no problem - until the next (handheld) shoot when I forgot to turn IBIS back on!

Now I do a 'take-of' check before a shoot and run through all the key settings on both cameras (X-H1) beforehand.

Another user error is to find that the (front) focus button has been accidentally moved so the camera won’t focus.

Motto: when using a tripod turn off the IBIS and check the focus button frequently in a shoot!

A more subtle point is that the focus square is very susceptible to high contrast or bright backgrounds so the target should fill the box AND you have to ensure it doesn't move slightly off when pressing the shutter. Also, make sure, if possible, you choose an area of the subject with high contrast e.g. glasses rather than a smooth cheek etc
