Help with group AF.

Hello, I'm having a issue getting two subjects in focus. The camera only seems to be focusing on one subject when I pose the subject behind the other subject. There's nothing wrong with my lens. I have already tested for decentering, front focus, and back focusing issues. I'm using the A7RIII with the 50mm F/1.2 GM. I have a flash mounted on the shotshoe. I decided to pair the subjects side by side. On AF-S mode one of the subjects was out of focus at F/4. On AF-C it was the same. I switched it to AF-A, and both subjects were focused side by side. When I had one of them pose behind the other one only one was in focus while the other wasn't. I have tried Flexible spot large and wide for focus area, and nothing seems to be working. I have also tried turning off Face/eye AF as I heard you're suppose to when taking group photos. That didn't make a difference. Everything is updated to the latest firmware. What setting could be causing this issue?
