I would challenge that in the pre vacuum era, and especially in current times where even the cheap bodies offer it as a feature.

I have in excess of 600 dives across many different camera systems from 2003 to current without a flood.  The Aquashot 20D saw 7 years of action, though there was a close call with the electronic sync port.

With a nauticam setup, the only time I've seen a flooding event at one of the photo workshops I've been with was when someone didn't put the cover on the push button valve and ended up depressing it during the dive.  I've happily stayed with the older twisting valve that doesn't have this vulnerability.

in Palau in October, I did see multiple floods on the Aggressor, but by people using lower end housings without that protection.   The compact housings are especially vulnerable.

The other key safety step is, I think, to not over fuss.  Set it up, lube everything at the start of the trip and then not again.
