Thanks Abbott, I have the 100-400mm f/4.5-5.6L IS II which is an EF L lens. I use Canon 6D and SL1 cameras, I have cleaned the contacts on lens, converter and both cameras to no avail.

It is about camera (6d), not lens + extender combination. See e.g. https://www.dpreview.com/forums/thread/3787861

I had the same issue when I tried that combination on my 700D. Strangely the Kenko Teleplus HD 1.4x DGX for Canon EOS EF/EF-s works on ALL my Canon lenses. However this is not a recommendation as I'm not overly impressed with the speed/ease of focussing nor the final image results.

EF 1.4x III is the best of Canon's converter series, but still has negative impact on IQ, especially with zoom lenses.

It only auto-focusses with higher end cameras which excludes my 700D.  That ruled it out for me after trying it so I returned it for the Kenko.
