Screws can work themselves loose from vibrations, especially vibrations from cars and airplanes.  I took a photo trip to Alaska a few years ago.  Once I got there I tried to mount my 500mm F4 lens to my camera body and it seemed wobbly.  I discovered that many of the screws holding the lens mount to the lens body had worked loose, almost to the point of falling out. Fortunately someone else on the photo workshop had a screwdriver so I could tighten the screws.  Now before every trip I do two things: tighten all the screws I can see on my gear, and ensure I am carrying along a screwdriver.

I have also had a screw inside a first generation 100-400mm lens work loose and jam the zooming mechanism.  So apparently these things happen.
