I'm shooting horizontally (assuming that this means my camera is upright like it usually is rather than face down) with a very primitive setup, basically consisting of a box in front of my computer monitor with 2 old VHS tapes, one to place the camera body on, and the other to rest the lens on and hold a cardboard negative holder which I taped onto the VHS cassette. This method seems to work fine but it can result in slightly out of focus regions of images.

I'm shooting at 50mm with a macro tube extension.
