You don't need high resolution for portraits. If you use Portra, it will print very well up to 14"×11", maybe even larger.

The camera should be small, with a small prime lens, preferably quiet. A big SLR with a bazooka-like lens would be too intimidating.

If decide to go medium format, don't use SLRs. Rolleiflex is a very good advice. If you can live with a waist level finder, of course.
