For lens I can highly recommend the following:

Zuiko 38mm f/1.8

Zuiko 40mm f/1.4

Voigtlander 40mm f/1.4

As far as body... If you can live with tight dynamic range, the Canon EOS M is tough to beat for vintage glass. With Magic Lantern it can magnify on half-shutter - invaluable for quick manual focus. It's also very compact.

If not open to that, an E-PM2 might be fun - still tiny but with IBIS.

Other lenses I never used personally but looked like good choices for this... Helios 35/2 (a cine lens) or Minotar 35/2.8 - these can be.found on ebay already converted to leica thread mount and are absolutely miniature.
