It is trial and error, basically. The best way to ensure proper alignment is to use some mounting hardware, such as a camera plate and flash bracket and secure the flash unit to the lens. Assuming a large telephoto lens.

With my rig I used half of the Wimberley F-2 bracket and an off-camera cord to mount the flash flush with the lens. Additionally, I secured it to the lens hood using gaffer tape. I was quite proud of my rock-solid contraption, which solved all alignment issues I had before and made handling easier. But soon I realized I did not like the unnatural reflection in the birds' eyes. Having the flash on the hot shoe was a bit better in this respect. Finally I gave up using flash in bird photography altogether.
