I'm sorry to learn this Carlos and know the horrible feeling as we watch our valuable gear fall to the ground.  The scratch on the TC may not have an effect on image quality and that's if it is on the glass.

As for the cause, we can only speculate and you are asking the correct questions.  The Fuji TCs have a rather tight fit when knew and can be hard to snap into place.  There should be a distinct click when it locks and you can see the difference in the position of the lens release button.  Mount the TC again and see if your are fully mounting the TC.  Also, the TC should be mounted to the lens and then the lens + TC to the body.
