Layout/Printing Options

The included image is a snapshot taken with my phone to illustrate the issue.
I've been using Lightroom for most of my work lately. For this particular task, I'm printing a 4x6 with the layout as shown for each of a bunch of insects. Lightroom is not very good at this sort of layout, or I haven't figured out how to do it. I printed the image on the left from Preview in OSX Catalina, where I've marked up the PDF output by Lightroom with the necessary text. That's easy to do in Preview, but I haven't found a way to print from Preview using an ICC profile. Is there a way to do that? If so, that may be my best option.
If I try using the Colormatch utility, it doesn't see the text I added in Preview. I wonder why. The right image was printed by exporting from Preview to a TIFF, then importing that into Lightroom. Then I printed from LR using the ICC profile. That's fine, but the text is not as crisp, probably from the export from preview.

Any suggestions on something that would get me what I want? The image will always be a square image at the top of a 4x6 card. Below that, the name of the animal in italic, larger font than the text below it, which is not italic. Seems like a simple enough ask. And I need to use the ICC profile.
Maybe this is simple to do in Photoshop? I've not used Photoshop in years, but I have it now.
