Must DSLR to Mirrorless Adapters Contain Glass Elements?

Me so confuse... I see available both simple adapters for EF lens to M-mount bodies as well as "smart" adapters costing many times as much from Metabones et al.

What is necessary for this adaptation but an empty extension tube of the proper length, preferably with electrical contacts and wires connecting those of the lens and camera body?

Is there more to this than simply achieving the proper separation of the the DSLR len's rear element from the mirrorless' sensor to permit focus at infinity?
