80D Constantly mis-focusing

I'm having some major issues with my 80D. It seems like all of my photos are out of focus. The camera seems to be focusing inches (to feet) in front of what I'm actually focused on. Like in every shot.

It's definitely not user error. My ISO is low, aperture at 4.5, and shutter speed is 250+

If I put aperture at 2.8, then nothing is ever in focus.  It's very frustrating.

In the pic provided, I had my (single) focus point right on his eye. The focus is like 2' in front of him. I have other pics where he's sitting, leaning on his knees. His face is always out of focus, and his hands are in focus. This happens with all of my lenses. Can this be fixed? Anyone have a link to a tutorial? Thanks so much!
