Look from the side.

The ground connector is in the slit of the adapter's foot.

This is the foot on a digital flash. It has some data pins in addition to the trigger pin.

The side connector of the flash or adapter connects to the rail of the hotshoe.

Notice how the paint has been removed from the rail where the ground connection is, to ensure a good connection.
