Thanks, of course possible that the body or adapter is buggy, but there is a confirmed front focus issue for the new RF 70-200 too.:

I had been under the impression that on-sensor focus conclusively solved front / back focus problems, but the RF 70-200 front-focus issue simply proved that not to be the case (although the implications of this don't seem to be well recognised).

My understanding is that lens focus mechanisms are not going exactly to where the camera (sensor) tells them to. It appears to be a potential flaw in the system that Canon have over looked. Will be interesting to see if the R5 has some kind of AFMA.
