back focusing with macro + diopter

I've been using a Raynox DCR-250 on my Canon 100mm macro (on a T1i DSLR), and it's been great to take pictures of tiny bugs, but I've been finding focusing really difficult.  It seems to be consistently off focus, so that when it looks like a bug's head is in focus through the viewfinder, it's actually focused on the thorax which is enough to mess up the image at that DOF.  I don't see why this should be; I've checked using the lens alone and it seems to be just right.  Maybe the diopter just exaggerates whatever tiny error there is, but it seems odd that there's no visible error at all with the lens.

Is there anything that can be done about this? Or am I doing something wrong?
