Shooting portraits wide open DSLM VS DSLR

I got a Z6 and I am super satisfied with focusing, from eyeaf and all the reliable focus points.

I want to get a D700 as a second body to experiment with some things, I never used a DSLR though. Focusing and recomposing shooting at 1.4 or even 2.8 seems to be really difficult, one thing I never need to worry about with my Z6 since I compose and move the joystick to the eyes or rely on Eye-Af.

So how do seasoned portrait photographers use their DSLR's with just 51 Af-Points to get the sharpest eyes / photos with open apertures? I reckon that composing will be quite limited and you still need to recompose often even when using the most outer points? Or do you use live view?

I've read that outer af points might be not that reliable. So that means as a DSLR-User you just need to shoot more?
