Focus problem?

Focus problem?

Just returned from a convention where I was shooting with the A7Riii.  In going through my photos, I find that about 60-70% of them are missing focus...  I am using focus peaking on the presenter's hair which I would think would be pretty accurate.  Instead, it focused perfectly in many cases on the background about 3-5' behind the subject.  Settings and equipment are as follows...

A7Riii

Canon 70-200L 2.8ii

Metabones V

f2.8 / 1/160s / iso1000

I'm truly surprised because I would think that the EVF would show a pretty exact representation of how the image is going to come out and it is dead nuts on.  Then the image produces and it's off quite a bit...  Could this a product of the adapted lens?  Is it potentially a microfocus issue?  If it's a microfocus issue, how does one resolve it if the EVF shows it as focused and how is it happening in MF???

Stumped...
