It is just that the adapter screws on only very slightly, and the weight of the subsequent lens makes it very easy for it to fall down.
The adapter uses bayonet mount, not screw; it turns about 90 degrees CW and locks with audible click, then it sits in place; I see no way for it to fall down (other than breaking either the adapter or the lens). I believe the original Canon adapter has to be mounted the same way; but from your description it looks different. Maybe some dirt is blocking turn? Did both "leafs" of the adapter mount get caught by on-lens counterparts?
Does anyone know of any alternatives to the FA-DC67A adapter?
The one I use was bought together with lens hood -- it's JJC LH-JDC100, I see plenty of them on ebay, each with adapter. About half the price of Canon adapter alone, and you can mount the lens hood and filter at the same time. There may be better ones, of course; this one just works for me.
I was particularly interested to know of any that could use the internal screw on the lens, instead of the one outside where the FA-DC67A attaches (which seems to be a very flimsy screw).
As Dugboy 1805 has stated, the lens has no screw -- these grooves just go around, they don't form a thread. I've seen metal filters forcibly screwed in -- it can be done, but I wouldn't damage my camera like that.
