I have also made this trip. I do not know, how sharp or good your 12-40mm lens is, but even if only o.k. that would be my first choice. If it is not that great, recommend another, better lens with the 12mm start, as many others have suggested. Then, I would take along your 42.5mm and an extension ring for close-up/macro shots, portaits, mild telephoto shots, etc. Finally, I would add a 50-200mm or 50-175mm, or maybe even a 100-300mm for distant mountain top shots, wildlife captures, etc. As others have mentioned, it is a once in a lifetime trip, so spending another grand or so on equipment is not out of the question, if you have it.  One more thought - most of your gear should be weather-sealed, since Peru can be very damp and wet!  Lastly, if you are into nighttime photography, then a wide aperture and tripod would also be required, since f1.7 or better are required to freeze stars at 1600-3200iso.  In any case, enjoy a spectacular trip...
