Filter causes problem on Pani 100-400

I recently purchased the Panasonic 100-400 / 4-6.3, including a Hoya Prime XS UV filter.
Unfortunately the images from the lens were disappointing: not sharp and a weird bokeh.
At first I thought the lens was defective, but a test showed that it was the filter that caused the problem.
I wonder, is the filter defective or is it possible that a certain combination of lens and filter does not work?
