Is this flash ghosting? How do I stop this madness?

I have no issue getting sharp photos...until I use on-camera flash. I have a Canon Mark IV, and I'm using a Canon 600EX-RT. I'm getting what looks like motion blur, but that doesn't make any sense, considering my settings. Regardless of shutter speed or aperture, this happens, and only when I use flash. I read an article about flash ghosting, which seemed to explain this phenomenon, but it was the only information Google found for me. This is a serious problem, and I need to both fix the issue and fix the images I've already taken in PS. So, I guess I need insight and advice for fixing two things. Thanks very much in advance!
