The OP is running 'Mojave' which means a Mac, so above which is Win doesn't apply!

On a modern Mac when using Application Color Management the Color Management settings in the printer driver SHOULD be greyed-out, i.e. unselectable and correctly set.

Is it possible that the bug with 16-bit printing in Mojave is kicking-in?
