I don't use flash much, but if everything was set up properly and the flash raised/turned on, then shouldn't the fire always fire? i.e. not dependant on lighting levels?

At the shutter speeds you were getting using f13 - yes.

In much brighter light situations the flash can be disabled by the camera in auto flash modes on the flash if the flash would lead to obvious over-exposure.

Using manual exposure and manual flash settings, the flash usually fires even in an over exposure situation.
