After analyzing, it seems the problem is that I have forgotten how incredibly shallow the depth of field is at 18 inches....That tiny sliver where the arrow points is all that is actually in. The rest is out. Brain cloud.

Any lens used wrong will be unsharp. I do believe that the photographer is the most important part of making sharp images, not the lens. I used to own the 24-70mm f/2.8G non-VR, and after thousands of images with it, I was able to handhold around 1/10 shutter speed pretty well.
