Nothing in focus. What happened?

Shooting basketball with my E-M5 Mkll yesterday. I was in S-AF mode and feathered the shutter to keep focus in the zone.

When this player spun to shoot, I fired off a burst of 4 shots at 10 fps. Every one of them was totally out of focus. Oly 12-40 lens, at f/2.8 and 1/400th shutter.

The focus square, on review, was on the players' shorts.

This happens to me occasionally with my Oly gear and I can never pin it down to a cause. It's almost as if the camera says, "Whoa! I wasn't ready for that!" But how can it physically get EVERYTHING out of focus?

I did get other useful shots of the game — but the blurred sequence would have rendered at least a couple of keepers.
