I have not been to Tanzania, but have been to the Masai Mara side of the Serengeti in Kenya several times (and will be going back again in another week).  I am a firm believer in having two bodies; my basic safari kit is two D7500 bodies with 18-140 and 200-500 lenses mounted and a 70-300 zoom as a backup lens.  I typically wind up with a loaded camera pack at 11kg and my clothing bag at 4kg.  If the safari flights in Tanzania work the same way as they do in Kenya, while your checked bag plus your carryon bag are not to exceed 15kg in total - they do not weigh items on your person.  So stuff some chargers with batteries in your pockets and carry a body with telephoto lens mounted on your shoulder.  That strategy gives you a little more leeway on the 15kg baggage limit.

PS - I would take the 12-40, 40-150, and 100-400 - there are scenarios where each of those lenses will be useful.
