Mostly handheld, and as you suggested, with the camera out in front of me. It's not that tiring with the 50 1.2L. With a 70-200 2.8, yea that's tiring, but any prime lens, its not bad at all. It's not the most ideal setup that I'd like, but it's sufficient.

I just got in to photography and I love the artistic and technical aspect of it.
