SD15 Autofocus Woes

I have a newly serviced SD15 with three lenses: Sigma 17-70 (non OS), Sigma 50mm 1.4 (non Art), and a much older Sigma 28-80 macro lens from an SA-9. I’m doing portraits up close (~6 feet) and needing the eyes to be sharp. I’m shooting at f5.6 or wider and I’m using a flash a lot of times (so shutter speeds no slower than 160 ).
Four out of five of my photos are sharp in the eyes. The others appear to be back-focused. It’s the same with all three lenses, but slightly worse with the 28-80. I’m not ready to dismiss this as a camera or lens issue. Honestly, I shoot mostly film and use manual cameras, but I’ve had to update my digital arsenal to do some senior portraits this fall.
What are some simple things I should check (settings or technique) to see if I can remediate this? I always place the AF indicator over the eyes or lips, half press, then recompose. AF is usually pretty responsive and doesn’t hunt.
