Does this occur only when you have these two filters stacked? If so, there is your answer.

I'd avoid stacking filters if at all possible and it I need/want to, I'm using quality glass that isn't likely to reflect back.

I'd also try to avoid small apertures like f18 is possible. Wide lenses and tiny apertures seem to find all sorts of issues and weak points.
