Why does my 5DsR sometimes underexpose?

My 5DsR is usually quite good with exposure, but sometimes it gets it horribly wrong. For instance, I shot a hundred or so photos recently with the 100 - 400 F4.5 - 5.6 II lens and exposure was good. I then switched to my Sigma 180mm F2.8 APO EX DG macro lens (a lens which never gave me any issues on this body before). Some photos were properly exposed. Some not.

Take this photo - why would the camera do this?  I had about 40 out of 70 photos totally underexposed.

I sometimes have the same issue when I hook up my MP-E65 macro lens and the MR14 LED ring flash - it would sometimes underexpose by 2-3 stops. Sometimes taking the battery out temporarily fixes the issue.
