What did I do wrong this time

Last night I went down to the MIT/Harvard campus area as the views of Boston are amazing there.

I have been reading a ton on here and watching tons of videos on the right settings.  So I take about 90 pics last night and I get home,  the ones I "thought" I got great ended up grainy.

I was shooting long exposure shots using a polarized filter shooting at F/9-F16 and shutter ranging from 5-30 seconds and ISO at 800-1600.  From what I am reading is at night,  you will get better results with a higher ISO.  I am confused/annoyed as I can't figure the "triangle" out.
