You haven't said much about what you want to do with the new lens, but one combination which definitely does work, and costs much less than the 200-400, is the 300/2.8L II with the two MkIII Extenders. I used mine with the 2x for three years before finally buying a 500/4L II, and that wasn't for image quality, it was for AF performance and greater maximum reach.

The 400 DO II looks like a very good option too, with the 1.4x when needed. I've handled one on a 7D2 and it feels great, but I've no experience of actually shooting with one.

Steve, to answer your question.. Umm, I think.. everything. Is that a valid answer?

I really like shooting people, and willdlife photography is certainly fun and rewarding, but probably not rewarding enough to justify the purchase of a $7000 lens.

I hear you on the 300 2.8. I'm sort of torn between a 200, 300, and 400 DO. I've read that the 300 and 400 DO are not that far off in terms of weight and size, with the 300 being a bit cheaper.

I've just always wanted a better 400mm+ option that is brighter than 5.6. So an updated 200 f2 + 2x teleconvertor, 300 2.8 + 1.4x or a 400 DO should do the trick.

Also, there's a rumour that the 300 will get an update soon. Hmmm... Luckily there is no big rush.
