Olympus LH-76 lens hood for 40-150mm f/2.8 Pro lens falls apart

Last fall Olympus replaced the lens hood for my 40-150 Pro lens. It simply fell apart when I extended it after less than a year's use. Now, less than six months later, the same thing has happened again. I pulled the lens out of its bag and found that a black ring had come off. When I extended the hood it exploded into an assortment of parts, including five little ball bearings and five little copper strips. So... if you find the black ring has come off, DO NOT extend the hood. There may be a way to re-attach the ring and save the hood from self-destructing.

I emailed Olympus Customer Care and after two weeks I have had no response from them. Has anyone else had this lens hood fail?  Anyone know how to re-attach the black ring so the lens hood won't come completely apart when extended?  Is it typical for Olympus to take more than two weeks to respond?
