Have you had a Compact Flash or SD Card Failure?

Just curious with all the newer cameras having dual slots and people now saying they NEED dual slots, has a compact flash (or SD) card ever actually failed for you?
I've put the answers in order, first round of questions is for total failures with low end cheaper cards, mid level cards, and professional / fast cards.
Next set of answers in the same order, but for less drastic issues such as corrupted / lost photos.
Last option is in the same order, but if you have not experienced any issues with your cards.
Feel free to share what your card was, brand and/or just size. I assume larger cards may have higher failure rates.
