Need some helps drafting a simple contract (Wedding)

Hi Everyone,

I have been working part-time as a wedding photographer for numbers of years until 2010. At the time, I was simply too busy, and didn't want to put any additional burden to my wife during the weekend, as she is also working FT with a baby and a small child.

However, every so often, I still have people asking if I can help photograph their or their friend's wedding, and I found myself having a hard time deciding.

One of the primary reason is the potential risk with a lawsuit similar to this:

I used to charge $1200-2200 a wedding back in 2010, since I don't plan on making living doing weddings, I just want to charge a lower fees to cover my time and equipment wears.  Can you guys/gals help me, so that I can draft a simple contract where I can set myself to be legally not being responsible to:

1> Being absents

2> Unable to provide any photos

3> Photos are crappy (low quality)

It's not like I actually shoot crappy photos, nor I plan on being absent.

But I just don't think I want to deal with or have any legal obligation when I am helping a friend's friend wedding while I only charge the person $500 to cover my time.

Please advise.
