Going from warm inside to cold outside is not the problem, it's bringing cold camera/lens into the warm humidified indoors air that creates condensation. Bag both camera and lens before bringing indoors, let them come to the room temperature before opening the bag.

Mostly agree. However, if the humidity is high, the "dew point" can result on condensation forming on the front element. That can especially be a problem if the camera is pointed upward.

My practice is to be overly careful by putting the camera + lens in a padded pack and let it acclimatize for several minutes before starting to take pictures. That's probably not all that necessary, and optional if time is a factor.

But, returning back inside that is definitely advised. I'll take the battery and flash card out of the camera before putting the camera in the padded pack. That allows me to start recharging the battery sooner, and start post-processing.

Also, I can check how the images came out, and perhaps go back outside for more captures if there is room for improvement.
