Exposure calculator app problem

Is anyone familiar with this particular exposure calculator app?

I've been using it lately but all the images are underexposed.

Here's my procedure for a long exposure shot:

I'll compose my shot on the tripod.

Manual focus.

Put it in aperture priority mode (at f/16 for example) and see what the exposure time is (without ND filter).

Put on 10-stop ND, and viewfinder block.

Calculate using the app, setting the EV adjustment to +10 for the 10-stop filter.

Shoot in Bulb mode using a wired remote for the time calculated.

Another dark image!

I'm particularly confused by the two orange buttons at the bottom/left of the app screen. Are you supposed to adjust the bottom two values to match the top f-stop and ISO or what?
