How to focus with a 10 stop ND filter

Ok, I've read all of the suggestions and still can't get focus using the 10 stop.  I've used filters a lot and it seemed straight forward except that you can't actually see through the lens after the filter is on.   I set up the shot and then gently slid in the 10 stop filter into my Cokin holder.  Then I switched to manual focus and hit the remote shutter release.  I can't get any of my shots to be in focus.  What am I doing wrong?  Do you think I'm ever so slightly moving the lens when I put in the filter?  I'm frustrated and about ready to step on the thing.
