Well when I set up my 4x5 the first thing I do is compose. The second thing I do is make multiple readings with my spot meter and then use that data to calculate my exposure. There is little more manual than using a 4x5 film camera.

Shooting manual has nothing to do with using a light meter. In fact the proper use of the light meter was the basis of the photography of people like Minor White, Ansel Adams, etc.

Using a good light meter - it might be in the camera - to estimate the exposure is the first step in most photographers. In some cases a photographers have the experience levels that they just know but then again - Adams, except in a few exceptions, used his meter.

Again, what I meant was not to ignore meters.  But using a reflective spot meter in the camera or even matrix and just lining up the +/- in the camera is not going to yield the best results.  You still have to use your head based on the subject.
