It is mixed.

Some romantic portraits where ambient light should do it only.. flash is forbidden. But mostly i work with flash but nearly 100% of usage is in bounced mode. Walls, ceilings, installations whatever is suitable is used. And if there is nothing i have a piece of white foam to act as reflector. Even some romantic portrays were done with flash as well but flagged against the subject and i dragged the shutter to control the ambient light. It is easy...

I nearly never use direct flash. Only outdoors sometimes to lift shadows In houseflash is always flagged agains the subject and lead thru a black piece of foam mounted to the flash thru a hairband.
