I always start with a RAW file, and open in ACR (CS6). I always use the color noise filter, typically with the slider at 15 to 25 - just enough to remove what is noticeable. I never use the luminance filter in ACR, because it removes way too much detail. No sharpening. I then open in PS, crop if needed, and save as TIFF. I then open the TIFF in Neat Image Pro and use the luminance filter only, and save as TIFF again. I reopen the new TIFF in PS, and will typically sharpen a little before resizing. If needed, I'll sharpen a little more before saving the final product.

Here's one shot at ISO 3200 with a 7D and Tamron 90mm w/1.4x TC, then two 100% crops showing before and after using Neat Image, with no sharpening:
