I don't use manual mode a lot, but. . .

It's the spot meter that's confusing you, if I follow. When you dial it in for one area and recompose, the results will reflect the exposure settings according to the new location of the spot. Since you're in manual, take the shot anyway - what you metered with spot initially should be reflected accurately despite what the display is showing when you recompose (assuming you don't have auto ISO enabled).

I work in CWA mode, and use the HL/shadow blinkies to assess the overall exposure. To protect the highlights I'll dial in to just barely set highlight blinkies to be evident, or a click below. If doing this and recomposing (in Av mode) I'll lock the exposure before recomposing. Working in manual metering mode, locking the exposure isn't necessary as manual mode is in essence a locked exposure.
