What makes you go "Doh!" ?

I was just reading how some forget to disable IBIS/OIS when using tripod (with mixed results).   I also was reviewing recent exposures, finding many shots ruined for another reason - forgetting to adjust the IBIS FL setting for adapted/manual lenses.   Funny, I have no problem with most other adjustments when going from one type of technique or exposure to another, just this.

Yea, it would be fortunate to have the camera be able to compensate for being less than fully engaged in the moment, but I don't blame the gear for such errors (though it seems to be a fairly common reaction to such op-errs).

So it got me to wondering what other frequent operator errors others might chide themselves over.   That is, what makes you go "Doh!"  ?
